Plants erect and up to 25cm. Leaves are overlapping, cauline (arising from the stem), mostly sheathing with purple bars at their base. The inflorescence is corymbose (flat-topped) with 1-4 flowers which are white to pale-pink with some purplish spots. The lateral sepals are broadly oblong. Spur 3-5mm long. It is thought that Disa fasciata uses floral mimicry to entice pollinators, it is very similar in form and colour to the more common nectar-producing China flower Adenandra villosa.
